Use this for to request non-acquisition items for purchase. You can request multiple items on a single form as long as the estimated amount includes all the items. After submission the form will go to the Director of Operations for approval. Approved requests will be ordered with the next office supply order. If you need the item(s) quickly please use the expedite check box.

This application is a supplement to the University of Missouri Application for Student Employment. This supplemental application application will not be considered unless you also complete and sign the University of Missouri Application for Student Employment.

All requests to start a recruitment effort or complete a hire for vacancies already in the recruitment process must
be approved in advance by Vice Chancellor Plain and Chancellor Dehghani. Recruitment should focus on the
following priorities: ensuring our students receive a high-quality education; continuing research, especially related
to the public health crisis, and supporting the University’s response to the public health crisis. While completing
