Library Account Proxy Management: Information

Any person with an active Library Account in good standing can authorize up to 2 individuals to act as their proxy. A proxy may check out materials for the account holder from the library. Account holders should use the form below to assign proxies or remove them. Any individual acting as a proxy must also have an established library account in good standing. Valid account holders are:

Registered Students of the University

  • Your library account is automatically created
  • Your library account number is your student number
  • Your library card is your campus issued student identification card

University Employees

  • Your library account is automatically created
  • Your library account number is your employee number
  • Your library card is your campus issued identification card

Other Members of the Public

  •  A library account will be created for you on request
  •  Your library account number will be the number found on an accepted form of photo identification
  • Accepted forms of photo identification are:
    • Valid Drivers License
    • Valid State Issued Photo ID
    • Passport
    • Military ID
    • Student Photo ID (Any Institution)
  • Your library card will be the photo identification used to establish your account 

Please note: Account holders, not their proxy, are financially responsible for any lost or overdue material. Replacement cost for lost materials is determined by current list price of the item plus a $25.00 processing fee.
